John Beauman, MD, FAAFP spent 13 ½ years as a US Army flight Surgeon. He is a senior AME and offers Class 1, 2 and 3 flight physicals as well as private and FAA Air Traffic Controller physicals.

If you are a private pilot, please go to https://medxpress.faa.gov/ and begin the flight physical process. When you are done please bring your confirmation number to your exam with you.

If you are an FAA employed ATC, please bring your FAA Form 3900-7 ATCS Health Program Report by Examining Physician and Invoice for Payment with you. You will need a full hearing test, so we ask you arrive 30 min prior to your appointment.

Please bring a photo ID with you to your appointment.

If you have any medical conditions that may require documentation, please bring all available information about those conditions from your physician with you.

If you have any FAA letters, read them and comply with its instructions. Bring them to your exam.
